About the event

Players are encouraged to attend all off-season camps, and clinics.  Any families in need of financial support for our camps should contact Coach Wayne directly at michael.wayne@fairport.org, or Jen Collins FairportFootballbc@gmail.com regarding our sponsorship program.

It is understood that the Fairport Football Booster Club in association with the Fairport Football camp staff, and all it's employees, are not responsible for accidents resulting in medical, dental and/or other expenses including but not limited to the loss of personal items. It is my intention that by submitting this document, the Fairport Football Booster Club in association with the Fairport Football camp staff and all it's employees be completely released and that I assume all risks to the extent permitted by law. The Camper must be in good physical, and mental health, and be able to participate in the physical activity of a vigorous program. In the event the camper needs to see a physician, or requires emergency hospital care, the parent/guardian's personal medical insurance is responsible for all costs incurred.

I certify that by submitting this form, my registered campers have been examined by a physician, have an up to date physical and is found to be in sound health to participate in football camp activities.

By submitting this form I am hereby releasing the Fairport Football Booster Club in association with the Fairport Football camp staff, all it's employees and the Fairport Central School District  from any and all liability for any injury, illness, or loss of personal items incurred by my child either directly or indirectly as a result of attending the camp.

The Fairport Central School district will not provide medical insurance for participants in the camp.

By submitting this form and registering my child (children) I give my consent to Fairport Football Booster Club,  and the Fairport Football Camp Staff in using their best judgement, to administer First Aid and/or transport to a facility for medical treatment if I or the emergency contact person cannot be reached.

Camp Registration Fee is NON-REFUNDABLE.

DANGERS OF CONCUSSION

Concussions at all levels of sports have received a great deal of attention and a state law has been passed to address this issue. Adolescent athletes are particularly vulnerable to the effects of concussion. Once considered little more than a minor “ding” to the head, it is now understood that a concussion has the potential to result in death, or changes in brain function (either short-term or long-term). A concussion is a brain injury that results in a temporary disruption of normal brain function. A concussion occurs when the brain is violently rocked back and forth or twisted inside the skull as a result of a blow to the head or body. Continued participation in any sport following a concussion can lead to worsening concussion symptoms, as well as increased risk for further injury to the brain, and even death. Player and parental education in this area is crucial – that is the reason for this document. Refer to it regularly. This form must be submitted by a parent or guardian of each student who wishes to participate in Fairport Football Booster Club Camps.

COMMON SIGNS AND SYMPTOMS OF CONCUSSION

- Headache, dizziness, poor balance, moves clumsily, reduced energy level/tiredness Nausea or vomiting

- Blurred vision, sensitivity to light and sounds

- Fogginess of memory, difficulty concentrating, slowed thought processes, confused about surroundings or game assignments

- Unexplained changes in behavior and personality

- Loss of consciousness (NOTE: This does not occur in all concussion episodes.)
